National Health Insurance Could Become Reality by 2028 as Antigua and Barbuda Relaunches Universal Health Coverage Programme
Government advances plans for a National Health Insurance Programme aimed at making essential healthcare more accessible and affordable while reducing the financial burden on households.

Antiguans and Barbudans could begin benefiting from a National Health Insurance Programme as early as January 2028, as the government moves forward with plans to make quality healthcare more accessible and reduce the financial burden of medical expenses on households.
The proposed programme forms part of a renewed push towards Universal Health Coverage (UHC), with the government seeking to establish a healthcare system that allows residents to access essential medical services without experiencing financial hardship.
The Ministry of Health, Wellness, Environment and Civil Service Affairs officially relaunched the initiative on Monday, October 12, during a Steering Committee meeting at the John E. St. Luce Finance and Conference Centre.
Health Minister Michael Joseph reaffirmed the government's commitment to developing a healthcare system that is accessible, equitable and financially sustainable.
“At its heart, Universal Health Coverage is about people. It means ensuring that every Antiguan and Barbudan can obtain appropriate, quality healthcare without facing financial hardship. That principle must guide our work,” Joseph stated.
Under the proposed arrangement, the existing Medical Benefits Scheme (MBS) would transition towards a National Health Insurance Programme, expanding access to essential healthcare services while reducing out-of-pocket expenses, particularly for vulnerable individuals and families.
Joseph explained that while Universal Health Coverage represents the broader national objective, National Health Insurance would serve as one of the primary mechanisms for achieving it.
The initiative will also examine how healthcare services are financed, managed and delivered to ensure the system remains effective and sustainable over the long term.
During Monday's meeting, discussions focused on establishing technical subcommittees responsible for examining healthcare financing, insurance benefits, governance, legislation and service delivery.
Their recommendations will help shape the proposed insurance framework, including which medical services should be covered and how the programme would be funded.
Joseph also highlighted the importance of public participation, noting that healthcare professionals, employers, workers, patients and community representatives must have opportunities to contribute to the development process.
The government intends to complete the programme's design and preparatory work by the end of 2027, paving the way for implementation beginning in January 2028.
The meeting was attended by Medical Benefits Board Chairman Sir Lionel Hurst, Medical Benefits Scheme Chief Executive Officer Kevin Silston, Permanent Secretary Hildred Simpson and other senior health officials.
The government will also build on technical work previously undertaken by the Health Economics Unit of The University of the West Indies to help guide the programme's development.
While acknowledging the financial and administrative challenges involved, Joseph emphasised that careful planning and evidence-based decisions would be essential to establishing a system capable of serving future generations.
The renewed initiative represents a significant step towards strengthening Antigua and Barbuda's healthcare system, with the ultimate goal of ensuring that access to essential medical care is not determined by an individual's ability to pay.
